We delivered a lightweight but powerful Excel Add-in that empowered frontline processors to instantly capture error instances at the point of occurrence.

Data was stored centrally in an MS Access database, enabling consistent, structured recording of error types, causes, and downstream impact.

  • One-click error submission with pre-defined dropdowns to ensure classification consistency
  • A central reporting dashboard showing Pareto (80/20) distributions of error types, root causes, and impact metrics. As you might expect, 80% of the errors originated from 20% of the sources.
  • A built-in feedback loop for operations leadership to triage recurring issues and deploy fixes.

This solution created a closed-loop between error capture, insight generation, and process remediation—without disrupting day-to-day operations.
